Edit a photo with Soft focus

#Open your image with photoshop

# Create a duplicate layer.

#Filter>Blur>Gaussian Blur

# Edit> Fade Gaussian Blur

# Take the Lasso tool with feather 10 px and draw
outline of the eyes.

# Take eraser with 31% opacity and 37%
flow and erase inside the selected area.

# Apply this erasing technique into eyes, nose and lips. So,
you will have a nice soft focused image.
